If you have not attended our show during the past two years, please print this form and mail, fax or e-mail it to us along with a copy of your resale tax certificate.  In place of a tax certificate you may submit copies of any two of the following:  (1) a recent supplier invoice,  (2) a voided company check,  (3) a yellow page listing or ad.
